INSURANCE

Dental Insurance is intended to cover some, but not all of the cost of your dental care. Most plans include co-payment, a deductible and other expenses, which must be paid by the patient. 

If you have dental insurance, please bring your plan information necessary to file your insurance claim. As a courtesy to you we will promptly prepare and submit claim forms to your insurance company. Your co-payments will be collected by our office at each appointment and any further amount not covered by your insurance benefits is due within 30 days of treatment. Questions involving your coverage should be addressed directly to your insurance company. 

FEES AND PAYMENTS

To keep dental costs down, we ask that you pay for office visits and treatment at the time of service. Cash, checks, Visa and MasterCard are accepted. At your initial examination we will provide you with a copy of your necessary treatment and the estimated cost. 

If financial arrangements are required for large treatment plans or there is a need for delayed payment, please call our office. We will make every effort to work out an acceptable payment plan to enable you to take care of your obligation.
